If I have a baby at Baylor Regional Medical Center Grapevine, do I automatically get a birth certificate?
No, the hospital will supply you with an envelope that contains a yellow application. The application must be presented in person or mailed in with proper photo identification and payment. A return envelope is also enclosed. Please do not submit the Verification of Birth Facts.

How do I get a copy of a birth certificate?
Applications for birth certificates can be downloaded from the website or filled out in the City Secretary's Office. The completed application must be presented in person or mailed in with proper photo identification and payment. Please submit completed application to the City Secretary’s Office, P.O. Box 95104, Grapevine, TX 76051.

What forms of payment do you accept?
We accept cash, checks, Visa, Mastercard and Discover.

What is the difference between a long form and a short form birth certificate?
The long form has more detailed information than a short form. Short forms are normally good for everything but getting a passport. However, we recommend you check with the entity requesting the birth certificate to see which form they need. The City of Grapevine can only issue long form birth certificates for a person born in Grapevine.

What type of ID do I need to show?
We accept current government-issued photo ID - driver’s license, state-issued ID card, prison ID, military ID, and passport.

How do I get a long form birth certificate for someone not born in Grapevine?
The City of Grapevine is a remote site to issue short form certificates for anyone born in Texas, but to obtain a long form you must go to the County Clerk or local registrar where the person was born. For a list of County Clerks and local registrars visit the Texas Department of State Health Services.
